Reference Laboratory Contracts: National Commercial or Regional Reference Lab?

This contract comparison benchmarks a national commercial laboratory against a regional reference laboratory, test by test and term by term. It converts every quoted price to a percent of the Medicare Clinical Laboratory Fee Schedule, then adds courier, interface, escalator, minimum-commitment and rebate terms to show what each contract really costs over its full term.

It helps hospital and outreach laboratories negotiate from data, decide whether to single-source or split their send-out menu, and weigh price against turnaround, consult access and service.

The tool above is a sample build with illustrative data. Client versions are built privately for each laboratory.

What it models
  • Fee schedule comparison by test, with price as a percent of Medicare CLFS

  • Benchmark grading against routine and esoteric pricing targets, with negotiation targets where a competing lab is cheaper

  • Contract terms: off-schedule pricing, courier, supplies, interfaces, escalators, minimums, shortfall penalties and volume rebates

  • Multi-year total cost for single-source and dual-source awards

  • Weighted service scorecard covering turnaround, pathologist access, courier, IT and billing

Frequently asked questions

How do you benchmark reference lab pricing?

Is a national commercial lab always cheaper? Often on high-volume routine testing, but regional and academic labs frequently win on esoteric and interpretive tests, and contract terms can erase a fee-schedule advantage.

Should we split send-outs between two reference labs? Only if per-test savings exceed the second lab’s fixed costs and neither split triggers a minimum-commitment penalty. The model calculates both.

What contract terms matter beyond price? Annual escalators, volume minimums, off-schedule pricing, courier and interface fees, turnaround guarantees, auto-renewal language and the billing model.
